Do you have knowledge of fraud, waste or abuse of City or independent agency resources and would like to report it to the City Council Auditor? Please fill out the form below and the information will be sent to an employee of the Council Auditor's Office (CAO) for review.
Note : Independent Agency includes JEA, JPA, JAA & JTA.
Once the form is submitted, the Council Auditor or his designee will review your report and decide how best to proceed.
While providing contact information is NOT required, it can be beneficial if the CAO requires additional information to perform a review. The CAO will protect your identity as required by Section 112.3188 of the Florida Statutes.
Any relevant information you might obtain subsequent to your initial submission should be reported via a follow-up email submission.
